Prior to your first consultation you will be sent a comprehensive questionnaire to complete online, in the comfort of your home. This form takes approximately 90 minutes to complete and we request that this is done at least two days in advance of the consultation to allow Dr. Pulnik an opportunity to review your information and prepare for your New Patient Visit.
